Unity Health Center collects information for the birth certificate and forwards this to the Vital Records Division of the Oklahoma State Department of Health. Additionally, Unity Health Center provides a complimentary birth certificate; however, certified copies of birth certificates can only be obtained from the state or local registrars, in accordance with Oklahoma State law.
