Hospital Admission Procedures

Unity Health Center’ s admitting office, located in the front lobby of the North Campus location, is the first stop you will make if you are admitted to the hospital or having any outpatient procedures performed. Even patients who have phoned in advance and pre-registered, will still need to check in at admitting.

Be sure to bring your social security card, identification (driver’s license), insurance card, any paperwork you have received from your doctor and your Advance Directives (living will, power of attorney or healthcare surrogate documentation).

You will want to leave all of your valuable – wallet, money, credit cards, jewelry and similar items at home or give them to your family members at the time of admission.

One of Unity’s patient services representatives will take care of your registration, which will include signing the appropriate consent forms, discussing financial arrangements and providing the hospital with demographic information (address, phone number, etc.) A financial counselor is available should you have questions about insurance or billing.

Outpatient Admission Procedures
If your doctor has ordered an outpatient test or procedure (mammogram, blood test, chest X-ray), please pre-register over the phone by calling Unity’s admission office at (405) 273-2270.

The day of your procedure or test, you will proceed directly to the Admission’s Office to sign all proper forms and will be directed the appropriate department.
